Instructions for Participants to complete symposia evaluations and to receive educational credit for pre-, main and post-conferences, as well as all satellite meetings
Beginning at 8:00 am on Wednesday, April 30, 2008 and for the next 90 days you may complete the online evaluation form and print your statement of credit.

Sign in by providing your first name, last name and email address in the required fields.
IMPORTANT NOTE: The sign-in information must match the information used during registration. That information can be found on any registration confirmation materials you may have. Login requires an exact match to your SAWC registration. Did you use a nickname or full name? Did you use your professional or personal email address? Did you add your professional credentials behind your last name?

Complete the evaluation form for the sessions that you attended.
Select the type of CME/CE statement/certificate that you need from the drop down list (dietitian, nurse, physician, pharmacist, physical therapist, podiatrist, etc.).
Claim and print your statement/certificate from your own printer.
Keep the certificate for your records. You will not receive another one.
You may return to the site for 90 days to reprint your statement/certificate of credit.
